Digital Dental Model Articulation

In Vitro Evaluation of New Approach to Digital Dental Model Articulation.

Abstract
PURPOSE: The purpose of the present study was to evaluate the accuracy of our newly developed approach to digital dental model articulation.

MATERIALS AND METHODS: Twelve sets of stone dental models from patients with craniomaxillofacial deformities were used for validation. All the models had stable occlusion and no evidence of early contact. The stone models were hand articulated to the maximal intercuspation (MI) position and scanned using a 3-dimensional surface laser scanner. These digital dental models at the MI position served as the control group. To establish an experimental group, each mandibular dental model was disarticulated from its original MI position to 80 initial positions. Using a regular office personal computer, they were digitally articulated to the MI position using our newly developed approach. These rearticulated mandibular models served as the experimental group. Finally, the translational, rotational, and surface deviations in the mandibular position were calculated between the experimental and control groups, and statistical analyses were performed.

RESULTS: All the digital dental models were successfully articulated. Between the control and experimental groups, the largest translational difference in mandibular position was within 0.2 mm ± 0.6 mm. The largest rotational difference was within 0.1° ± 1.1°. The averaged surface deviation was 0.08 ± 0.07. The results of the Bland and Altman method of assessing measurement agreement showed tight limits for the translational, rotational, and surface deviations. In addition, the final positions of the mandibular articulated from the 80 initial positions were absolutely agreed on.

CONCLUSION: The results of our study have demonstrated that using our approach, the digital dental models can be accurately and effectively articulated to the MI position. In addition, the 3-dimensional surface geometry of the mandibular teeth played a more important role in digital dental articulation than the initial position of the mandibular teeth.

Despite claims that all marketing is headed for the web, printed marketing materials can be crucial to your small dental lab business. Brochures, business cards and other printed materials can help you stick in a prospective customer’s mind in a way that a website may not. After all, it’s easier to keep a business card than it is to keep track of a business website. The importance of your printed marketing materials means that you need to make sure that they’re high quality. In some cases, that can mean paying extra to get the best printing — but if you know the facts about getting your marketing materials printed, you can often find better deals with better printers.




However, knowing what’s important when it comes to using a printer can be rather tricky, especially if you’ve never had anything printed before. In order for you to understand what’s involved with the printing process, here are 5 facts you need to know.

1.Design is an add-on: No matter what printer you work with, you’ll find that his or her primary business is definitely not design. 3 Traits that will make you a more successful dental lab owner and entrepreneur

good advice can come when you least expect it — like when you're getting your hair cut.

Joan*, the hairstylist giving me a trim, mused aloud about what she was planning to do with her career. Cutting hair was just one part of her livelihood; she was also a professional caregiver as well as the owner of a rig that her husband operated. But her husband was about to retire from the road, and now they were wondering, "What next?"

Over the course of our brief conversation, in no more than the time it took Joan to cut my hair, I picked up on three attributes of her success that are helpful for any entrepreneur:

Practical. Listening to her brainstorm reminded me that successful entrepreneurs know how to keep their feet on the ground. First, they get inspired through personal observation, developing ideas from needs they see in the world around them. Second, they develop a concrete plan. They may work the plan, changing it as they go, but always with an eye towards getting a good return.

Purposeful. People with a practical outlook seek opportunities that add value, as opposed to opportunities that just seem "cool." (It's easy to forget this distinction, especially in well-established organizations.) Their focus is offering products and services that customers need and will pay for. For instance, Joan's second job as a caregiver: that's a service for which there is always a need.

Impatient. Sure, patience is a virtue in some cases. But for an entrepreneur, so is impatience. Joan is eager to make things happen so that she can continue to earn a good living. When it comes time for her husband to leave the trucking business, she will be ready with another venture. Her gumption and ambition make her impatient for success, and that drive increases her chances of getting there.

There's one final trait that successful entrepreneurs share: They realize that inspiration is useless without perspiration. During my 15 minutes in Joan's chair, we talked about three different industries she's involved in — personal service, health care, and transportation. People who work for themselves have to rely on their own get-up-and-go.

People like Joan enjoy working for themselves because it affords a level of independence. With hiring still sluggish at large firms, I suspect we will encounter many more such entrepreneurs. The future of our economy may indeed depend upon such folks, whether they are running a company that cuts hair, or running a company that makes microprocessors.

People who can think and plan ahead, are comfortable with uncertainty, and have the discipline to work hard are an asset to any country — and we ought to find ways to invest in them.

Expanding possibilities for labs and dentists
Laboratories using 3Shape Dental SystemTM can now add to the range of their services for dentists and other labs, significantly increasing the scope of their business and growing their customer base.

Dentists using the Sirona CEREC intraoral scanner can send their digital impressions to the large install base of laboratories with 3Shape's Dental SystemTM, significantly expanding their clinic's CAD/CAM possibilities and the range of their restoration indication options.

Broadening equipment flexibility
Last year, 3Shape and Sirona released a corresponding interface that enabled 3Shape scans to be designed in Sirona InLab® software and this new solution completes the circle. Complete case information transfer
As before, dentists send their Sirona CEREC Intraoral scans to the lab's InLab® system via the CEREC Connect portal. The lab can export both CEREC scans or InLab® scans from their InLab® system using a new dedicated ".3se" file format, designed to be loaded directly into Dental SystemTM for fast and efficient design. Transferred data provides all case information including the model data of preparation and antagonist, the patient's data, case relevant tooth-numbers and the requested type of restoration (crown, bridge, etc.)

The solution requires an enabling of the interface module in the 3Shape as well as the Sirona software. Users who wish to benefit from this opportunity should contact the supplier who provided their specific system for explicit information about availability of the interfaces for their system and how to obtain the correct software modules. Is the proliferation of digital technology having an impact on the industry’s remake factor?

Remakes 101
Mention “remakes” in a room full of laboratory owners and you know you’re in for a spirited discussion. It’s no wonder: not only do these cases-gone-wrong cut into your profits, but they can wreak havoc with your production schedule and erode the relationships you’ve built with your clients.
Add to that the frustration expressed by technicians who feel the bulk of remakes are beyond their control; in fact, our survey participants say more than three-quarters of remakes in their laboratories are due to dentist error. These laboratory owners and managers repeatedly say that—across all departments—most remakes can be traced back to an inadequate impression.
Of course, the quality of the final impression is dependent on a number of variables but what’s equally important is how cooperative the doctor is when problems are brought to his attention. While most of our survey respondents say dentists appreciate a call about inadequate impressions, they are divided on the most common outcome: 41% say the dentist generally sends a new impression and another 41% are usually told to “do the best” they can.
To drive the point home and document potential problems, many laboratory owners are taking advantage of digital communication, saying a picture is worth a thousand words. “Providing dentists with photos of inadequate impressions enhances communication and eliminates the blame game,” says Jessica Birrell, CDT, Owner, Capture Dental Arts, Saratoga Springs, Utah, who annotates digital photos with captions and arrows using Photoshop software and e-mails them to the dentist. “Sometimes, I’ll also pour a stone model and send photos of that to point out specific problems. If the doctor still wants me to go ahead and guess at margin placement, I let him know I cannot guarantee the success of the case.”

No One-Size-Fits-All Policy
More than half of all laboratories don’t have a defined remake policy, saying that it depends on the specific case and sometimes on the specific dentist-client. “When a case is returned, we have to determine the reason and handle it accordingly,” says Ross Gaiteri, Owner, Benchmark Castings, Inc., Columbus, Ohio. “If the lab is at fault, there’s no charge. If the fault clearly rests with the dentist, we charge 50%. However, if the dentist is repeatedly at fault for remakes—or we’re told to proceed when we call with concerns about his impression or model—we charge him full price.”

In an effort to eliminate the blame game altogether, 17% of participants routinely split the cost of remakes and one quarter of them don’t charge at all for remakes, at least until a certain remake percentage is exceeded. (Click here for a closer look at participants’ remake policies.)
To charge or not to charge is an issue that often divides laboratory owners. Some say laboratories shouldn’t routinely accept blame for cases that fail through no fault of their own. “If you don’t enforce a remake charge, it gives the dentist the impression that he can send you an inadequate impression or preparation at no risk to himself and therefore, there’s no incentive to do a better prep or impression,” says Marc Posen, CDT, Owner, Posen Dental Laboratory, Birmingham, Michigan.
On the other side are laboratory owners who perceive free remakes as one of the value-added benefits they offer their customers. In most cases, they say, the cost of doing the remake is negligible given the value of a good account. “I have only a few clients and very low remakes, so my philosophy is that doing the remakes at no charge is part of the service I’m offering them,” says Paul Francoeur, Owner, Rogue Dental Solutions, Inc., Ormond Beach, Florida. “When we do have that rare problem with a case, we communicate and work together to solve it, and they appreciate that I will take care of remaking it.” Of course, this approach requires laboratories to carefully monitor clients’ remake percentages and immediately address any issues.
The bottom line: whether you have a defined policy or not, keeping an eye on your clients’ individual remake factors and having honest conversations with them when things go awry is your best bet. “We know our mistakes cost dentists chairtime just as their mistakes cost us labor, materials and transportation costs. But we’re making a custom product for sometimes challenging, difficult patients and the partnership requires both parties to be reasonable in their expectations,” says Jim Thacker, Vice President, Utah Valley Dental Lab, Provo, Utah. “A flexible, relationship-driven approach builds trust and value for both the laboratory and dentist.”

The Impact of New Technology
Is the proliferation of digital technology having an impact on the industry’s remake factor? One effect is clear: two thirds of laboratories that receive digital impressions say they result in fewer remakes than do conventional impressions. “The software for these systems tends to ‘force’ dentists to provide accurate reduction, enabling us to fabricate more ideal restorations,” says Jim Thacker, Vice President, Utah Valley Dental Lab, Provo, Utah. “Models created from digital impression technology are extremely accurate and allow us to create more consistent, predictable restorations for our dentists.”
Milling, too, has had a positive effect although to a lesser extent. While the majority of participants—67%—say it hasn’t changed their overall remake percentage one way or another, one quarter of laboratories say milling has caused their overall C&B remake percentage to decrease due to more accurate fits.

Spray-free and easy to use
One of the most notable TRIOS® features was that scanning does not require dentists to apply spray to coat the patient's teeth, making scanning with TRIOS® a fast and patient-comfortable process that doesn't ruin scan accuracy by adding material to teeth surfaces.

TRIOS demonstrates accuracy and speed
3Shape introduced its new Ultrafast Optical Sectioning technology that utilizes up to 1000 3D pictures to create true geometries, based on real data, rather than artificially forming interpolated surfaces as seen on too many other systems. The scanner captures over 3000 2D images per second, which is 100 times faster than a conventional video camera.

Watching the impression being built on the screen
The cart's large bright screen displays a real-time 3D visualization of the impression being built up during scanning. TRIOS® images remain in sharp focus while moving the scanner across the teeth, and the tip can be rested on the teeth during scanning. 3Shape demonstrated a smart TRIOS® feature - a motion sensor interface that enables the hand-held to virtually rotate and turn the digital impression like a gaming controller.

Strong software support
The TRIOS® user interface is very clean and intuitive, and the large Smart-Touch screen makes operation handy for dentists. Large buttons take the dentist through the complete workflow starting with order creation and ending with dispatching the digital impression to the lab. The software contains state-of-the-art technologies including alignment of occlusion and clinical-validation of impression quality before sending to the lab.

Unlike many other intraoral impression-taking solutions, 3Shape does not attempt to bind digital impression taking with chair-side milling. The Medical Devices Bureau of Health Canada has recognized four classes of medical devices based on the level of control necessary to assure the safety and effectiveness of the device. Class I devices present the lowest potential risk and do not require a licence. Class II devices require the manufacturer’s declaration of device safety and effectiveness, whereas Class III and IV devices present a greater potential risk and are subject to in-depth scrutiny. [3]. A guidance document for device classification is published by Heath Canada [4].

As you can see saving $10 or even $3 on a coping that is not legal for use in Canada, as you can see, might not be the best route to take short or long term.

Dental industry takes aim at gray-market practices

In these tough economic times, everyone's looking for a good deal. While dentists may be tempted by ads in dental magazines and on the Internet offering products at discounts of 10% to 50%, they should be aware that the old maxim very likely applies: If it seems too good to be true, it probably is.

Deeply discounted products are often illegally imported gray-market goods or even counterfeit materials that could endanger patients or expose dentists to legal liability for using dental materials that are not approved for sale

GC America "What if these discounted products are expired, altered, or even counterfeit, and you have a material failure -- such as postoperative tooth sensitivity -- in a group of patients?" Gordon Christensen, D.D.S., M.S.D., Ph.D., recently wrote in the Journal of the American Dental Association (June 2010, Vol. 141:6, pp. 712-715).

Gray-market products -- often zirconia, ceramic materials, composites, and glass ionomers -- are showing up more and more, according to industry experts, and dentists may not even know they're using them.

For example, most manufacturers offer some type of zirconia, which is used in ceramic restorations and implants and can be obtained from only a few sources in the world, according to Bennett Napier, co-executive director of the National Association of Dental Laboratories (NADL).

"If the price point of the raw material is significantly lower, then there's got to be a question asked: 'How did you do that?' There's not a lot of price variation in the marketplace," he said in an interview with DrBicuspid.com. "It's a supply-and-demand issue."

And he believes the gray market for dental materials is growing. "I think more dentists and labs are looking at the ability to compete by cutting operating expenses and may be looking at other suppliers that they haven't worked with before. You have to do your due diligence."

How to spot counterfeit products

Potentially gray-market or counterfeit dental supplies can be identified in several ways, according to Dr. Gordon Christensen (Journal of the American Dental Association, June 2010, Vol. 141:6, pp. 712-715).

To avoid the problems potentially related to purchasing gray-market or counterfeit products, Dr. Christensen recommends:

Brand-name products are sold at lower prices in Latin America, Eastern Europe, or Asian countries than in the U.S., said John O'Neill, vice president of sales for dental equipment manufacturer GC America. Unscrupulous dealers then buy the materials in these countries and illegally import them into the U.S., selling the gray-market products at reduced prices.

"Many, many distributors sell products in the U.S. and Canada that don't have that product line," he told DrBicuspid.com. "If you don't buy from an authorized distributor of that manufacturer, then you're not really sure what you're getting."

Unscrupulous distributors are savvy about copying brand-name products, making gray-market or counterfeit products hard to detect, O'Neill said. "They're very sophisticated; they know how to tamper with packages, put on fake bar codes, or hide bar codes. There are a million tricks they play."

The quality of illegally imported products may have been jeopardized because of expired shelf life, improper storage, or poor handling. "When it sits on boat for two years, then sits in some garage, you really don't know what you're getting, and nobody can guarantee quality," O'Neill said.

He estimates that the gray market is a multimillion dollar business.

GC America recently won lawsuits against AR Dental and Kings Two Dental Supply that charged the companies with improper importing and distribution of GC America products. Both companies subsequently agreed not to sell, distribute, or import unauthorized GC America products.

"These suits demonstrated that these actions are wrong and we can take action against people who are selling our products that are illegally imported," O'Neill said.

Fighting back

The dental industry is regulated to ensure that the dental products used and placed in patients' mouths -- considered to be medical devices by the agency -- meet accepted standards.

Gary Price, CEO of the Dental Trade Alliance, which represents 200 manufacturers, distributors, and labs, said he has discussed the issue with the FDA and is producing a report for the agency on how to combat the proliferation of gray-market and counterfeit products.

"We want to get them to at least make pronouncements about the need to be careful about these products," he said, adding, "dentists bear some responsibility, too."

Some states have passed laws requiring the disclosure of consumer product information, including queries about where products are obtained and what ingredients they contain.

Sometimes dentists aren't getting what they paid for, and the material doesn't last as long because the quality is lower. For example, high noble alloys, which last longer and are made of stronger material, are more expensive than base alloys, Napier said.

He pointed out that the two classes of alloys are also tied to insurance reimbursement.

"If somebody asks for high noble but gets base noble, they get a higher insurance reimbursement. But if somewhere in the supply chain base noble is substituted, it could result in insurance fraud," Napier said. Although he has seen such cases, they are hard to uncover because it's difficult to track down the source, he said. "It's a needle in a haystack."

Disreputable distributors gyp both the dentist who's not getting the premium quality he paid for and the patient who may have to get a replacement sooner than necessary.

"Each material has a longevity that it's expected to last in the mouth," Napier observed. A dentist may tell the patient that the restoration will last 20 years, but if something inferior was placed, it may only last five to 10 years.

Patient risks

Counterfeit materials that haven't gone through proper protocols and testing could fail in the mouth, resulting in cracks or leaching of material in a patient's mouth, according to Napier.

"It may not jeopardize a patient's life, but you've cost them money and inconvenience so that's an important thing to consider as well," Price said.

He cited several factors which have contributed to the growing gray market: increased foreign trade, the North American Fair Trade Agreement with Mexico, the bad economy, and the fact that many manufacturers are more restrictive in authorizing distributors to carry their products.

The NADL, the ADA, and dental manufacturers have pledged to work collaboratively to make sure dentists know what questions to ask and what to look for to ensure they're getting what they asked for, Napier said.

A 2008 ADA survey on lab issues found that more dentists are seeking information on the source of dental products because patients are asking for it, mainly because of media coverage of dangerous levels of lead in imported crowns, Napier noted.

"This is healthcare, not a pair of socks," O'Neill said. "This stuff is going to be used in peoples' mouths. These are medical devices that are legally regulated by the government."

I am meeting with a few Canadian CAD industry leaders tomorrow - discussions based around forecasting the transition into digital, new products for 2011, current state of the industry, demographics and the increasing rate of dental technicians retiring.

Should be some great conversation. (and coffee)

Today I was lining up industry contact meetings
The Industries `Big Guns' are here - I have to see what I can get for you guys! (it won't be a free burn out furnance)but -
Information and Support in Canada - what are the stratigies for 2011 and beyond. The idea is then to align yourself with those `ideals' from very succesful vendors and work hard to create a very profitable year for yourself.

Tomorrow I am putting together my plan to source out the best vendor exhibits and capture all the best product tips and tricks for you.

I also have to pick up some Tanaka Big Brushes or I will be in big big trouble - although we don't touch ceramics inhouse - Our clients love these brushes. I said I would get a few! If you want one let me know

I am back from Chicago and very energized to provide a quick summary of what I saw and heard during my trip for you! gathering information and meeting clients and vendors.

Dentists - McCormick
On the Dentist side I did not see the big releases and new product launches you would expect. (I thought)
At McCormick the exhibits were absolutely stunning and mega dollar. The place was jammed daily just nonstop. I was able to locate a few ground breaking products - a free offering to your dentist partners from us to you (will talk you one at a time) Glidewell was pushing BruxZir as the next big thing – well they have now added their own IOS intraoral scanner for dentists – Interoral scan direct to manufacturer and get the models back with a e.max CAD for just over one hundred bucks. Well we feel the current solution is not as accurate as Itero or C.O.S – The models were off the Eden which we did not feel were accurate enough at the time. IPS E.max had nothing new other than they acknowledged to ask the dentist for a 2.mm prep and you might get a 1.2mm. The representative from Ivoclar did mention the strength under 1.2mm is far less and at .8mm pushes the limits of the material. Sironia had a big display and just system upgrades. Nobel the same – better marking software and release of the e.max CAD which has already been used for years with little traction because of margins and – well milling glass with square preps and round burs is a tough fit - there is no shrinkage like other products - Try it.

Over the to the CAL LAB Day
Jim Glidewell, CDT: “The Digital Future of Dental Laboratory Technology” (Fri. a.m.) – Lit Up the Scorched Earth Policy on conventional dental labs. Although I did not make this one! Almost every person I spoke to including senior staff at Glidewell - the voice was clear and heard, that if you don’t have or get yourself an open scanner your days are numbered, you will just not be able to compete in the near future. Jim then went on the quote just mind boggling production numbers like 8400 BruxZir units being milled each week and $10M in milled temp revenue each month alone – people were talking! Stated volume is slowly growing with e.max but BruxZir has shot through the stratosphere!
The Scanner Shoot out went as expected, heavy 3Shape. There were other great speakers as well.
Thanks also to the Plug from Mike (from the USA) when he was completing his key note speech and mentioned Kastle Mills being a great outsourcer! (I got a text I was on the other part of Chicago)

The Sheraton Dental Lab Days – was just packed!
Too much to cover. However it was all about CAD and all about Full Contour Zirconia. It was everywhere. New improved dental 3D scanners, BruxZir BruxZir and more BruxZir. I came across what we required for our clients a set up various alignments to access specialty products to help out dental labs drive out even better products. Nothing really new from dentalwings and like above little from Nobel.

What we did see is a crazy new zirconia from 3M ESPE launching soon against BruxZir – It was actually compared physically to Crystal and BruxZir I believe in a light chamber – you had to pick. It was obvious. The crazy thing is the wear study is complete; the zirconia is fluorescent and comes in over 30 shade configurations. – I was told even our milling systems would be updated for enhanced anatomy.

Another crazy new update was the enviosionTEC DLP – The models were far more aesthetic and clean than the Itero or 3M In’tech. WOW! I knew they were coming- its here. We are working on a solution for our Canadian customers. Keep you posted. Zeno had a one pack ceramic that did draw attention, 3Shape signed on with Atlantis for late in 2011.
Jenson officially joined with 3M ESPE in the USA only, seemed to be many relationships forming Invisionline and Itero and many more!

Is Asian OutSourcing DONE?
There was a lot of late to market ideas – such as Asian Outsourcing, at $39.99 a unit for copings and $129.99 Abutments Ti – Turnarounds 5 to 7 business days and that’s with no customs issues. It’s done! I have to offer $39.99 a unit Free Shipping and 48hours in your hands! – The Asian piece is not even competitive in price, quality and or speed. (5 years ago that price would have been turning heads to China)
Also milled models - $40K for machine and $20 to mill a set. Looks great but printed models are about $3 to manufacturer in materials. – There was a lot of this. late to market

There were a lot of intraoral scanners coming out (export STL) – It’s not the scanner you have to watch out for it’s the speed of placement as there are many scenarios I found - like Free Scanner and Pay for Click fee! Or Buy for $20K and no click fee! (It's Coming)

Overall in Summary it was 4 long days, of fun, new products, some impressive, some not. The theme was driven digital by most, great speakers, most with older studies – the presentations would then contradict the next speaker (had a newer study) – you get the point. The reality is that dentists are becoming more educated on materials, zirconia is not only here to stay it was a fairly large part of most of the dental lab day in one way or another. With new products being created to make zirocnia better, faster and even more accurate. Another talking piece was also the new printed temps! – coming soon. Well I am just skimming the surface here, I heard little about titanium at all, Vericore and 3I seemed to be stepping up and maybe capture some bar work from Nobel (who knows). I bumped into Matt Roberts literally at the 3Shape launch and I think stepped on Ed McLarens foot at the bar at the Sheraton.

Take Ways – from Chicago besides another 5lbs.
1. Glidewell – put it out there! the theme of the show – all I heard about – Buy an open scanner or you can’t complete (I will try to get youtube)
2. Become a modern technician now – (3Shape Tais CTO)
3. Be flexible and accurate with product offerings (Business)
4. Sell 2 brands of YOU, conventional and CAD Products – charge accordingly right! (CAD Mentors)
